Click on the tab register grievance and you will see one big form which is quite simple to understand but some of the few points need to be taken care of. Below is the screenshot of the form.
You need to fill up the mandatory fields and the first few fields are very important. First fields ask to select status, normally you will be PF Member but you should know your PF number which is provided by the employer. If you are not aware of PF number then you can select the others option in which you do not require PF number.
Next is the detail of the PF office in which your company is registered. These details are present with the employer and you can ask your HR for the details. You need to first select the office to which the grievance is raised. Next field is Name of the establishment which is your company name and in the address field you need to fill the address of the company.
